Thank you for your interest in the 2019 AMWHO at UNC Regional Conference University Delegation registration!

In order for members of your delegation to receive the University Delegation discount, the chapter president, or the delegation leader, should fill out this form with the first and last names of each member of the delegation, after those individuals have registered for the conference. Each delegation member registers separately before submitting this form and once this form has been completed, delegate members will receive a reimbursement of $10. The University Delegation discount is $10 (from the $45 early-bird registration fee or from the $55 regular registration fee).

If a delegate is registered, but is not included on this form completed by the delegation leaders, they will not receive reimbursement for the 2019 AMWHO at UNC Regional Conference. Your delegation must be a total of six or more individuals. These individuals have until January 6th, 2018 to register. This application is due by December 16th, 2018. No group registrations will occur after this date.

Please note that after you complete this registration form, each delegate will receive the $10 reimbursement after registration closes on January 20th, 2019.

Consequences
Your delegation must be a total of six or more, as provided by this form, that have already registered for the 2019 AMWHO at UNC Regional Conference. In the unfortunate event of your delegation size reducing to less than six, you will have until January 6th, 2019 to recruit enough members to reach the six member mark. Otherwise, every member of the delegation will not receive the $10 discount. If your delegation drops below six members between submission of this form and January 20th, the university delegation discount will not be applied.

Important Details

Processing fees and tax will be an additional charge.
Registration fees pay for most meals (breakfast, lunch, and dinner) for the three days, renowned speakers, delegate materials, and more.
Days of the Conference: January 25th to 27th, hosted by the University of North Carolina at Chapel Hill.
